Forms
Replace paper clipboards with digital intake forms on your kiosk tablet. Collect customer information, consent, and signatures — all saved automatically.
What are intake forms?
Intake forms let customers fill in information on the kiosk tablet before their appointment or service. Instead of paper forms on a clipboard, customers answer questions on screen. This works well for health questionnaires, consent forms, allergy checks, and any information you collect before a service.
Creating a form
Go to Forms in the left menu and click New Form. Give the form a name (for example "New Client Intake" or "Patch Test Consent"). You can add an optional welcome message that customers see at the top, and a thank-you message they see after submitting.
Adding fields
Inside the form, click Add Field to add a question. Choose the field type (see below), enter the label — the question the customer reads — and mark it as required if you need an answer. You can drag fields to reorder them.
Fields can also have help text — a smaller note that appears below the question to clarify what you're asking.
Field types
KlevaKiosk supports the following field types:
Short text
A single line for a short answer — name, phone number, email.
Long text
A larger box for notes, requests, or longer answers.
Checkbox
A yes/no tick box — great for consent questions or opt-ins.
Dropdown
A list of options for the customer to choose from.
Date
A date picker — useful for appointments or date-of-birth fields.
Signature
A finger-draw signature box on the tablet screen — for consent forms and waivers.
Publishing a form
When your form is ready, click Publish. Only published forms appear on the kiosk. You can unpublish a form at any time to take it offline while you make changes, then re-publish when ready.
To show the form on your kiosk, go to your station's Tiles tab, add a Form tile, and select the form. See the Tiles guide for more.
Viewing submissions
Every completed form is saved under Forms › Submissions. You can see each submission with a timestamp and all the answers. Signatures are shown as an image.
To export all submissions, click Export CSV. This downloads a spreadsheet you can open in Excel or Google Sheets, with one row per submission and one column per field.
Email confirmations
If your form includes an email field, KlevaKiosk can send the customer an automatic confirmation email when they submit. Go to the form's Settings tab and enable Confirmation email. You can customise the subject line and message body.
Tips
- Keep forms short — customers are standing at a kiosk. Ask only what you genuinely need.
- Use the signature field for consent and waiver forms to make them legally binding.
- Add a clear welcome message so customers know what the form is for before they start.
- Test your form yourself before using it with customers — fill it in on a tablet to check the layout.
- Export submissions regularly as a backup, in case you ever need to refer back to them.
